• 1. Subtype: Banking information (ACH to US banks only)
•   a. W9: (must be IRS version Rev. October 2018 or Rev. March 2024) signed and dated by company representative
•   b. Bank letter per BPX Requirements:

  • ii. On your company letterhead
  • iii. Signed and dated by an authorized company representative (i.e. CEO, Controller, etc.) within 6 months from submission date
  • iv. PDF format
